- I am fully aware of risks and hazards connected with shooting firearms and participating in the Activities with OA Defense and I am fully aware there may be risks and hazards unknown to me connected with being on the premises and participating in the Activities. I hereby elect to voluntarily participate in the Activities, to enter upon the premises, and to engage in Activities knowing that conditions may be hazardous or may become hazardous or dangerous.
- I voluntarily assume full responsibility for all loss, property damage, or personal injury, including death, that I may sustain, and any loss or damage to my property, as a result of my being a participant in or observer of the Activities, whether cause by Releasees’ negligence or otherwise.
- I agree to indemnify and save and hold harmless the Releasees, and each of them, from any Claims, whether caused by Releasees’ own negligence or otherwise.
- This release shall bind me, my insures, my subrogates, any minor child named below, as well as the members of my family and my spouse, my heirs, assigns, and personal representative, and shall be deemed as a release, waiver, discharge, and covenant not to sue the Releasees.
- I certify that I am not prohibited under a Federal or State law from possessing a firearm.
- I hereby consent to and authorize the administration of emergency medical treatment and waive any claim arising from refraining to administer such treatment.
- I have no physical or mental illness that precludes my participation in Activities in a manner safe for myself and others.
- I am not under the influence of drugs or alcohol which impairs my ability to maintain my safety or which endangers others.
- No oral representation, statements, or inducements, apart from the foregoing written agreement, have been made.
- I am at least eighteen (18) years of age and a fully competent adult, who shall be solely responsible for my safety and who also executes this Release on my behalf and on behalf of any minor child named below.
- I execute this Release for full, adequate, and complete consideration fully intending to be bound by same;
- If I breach the terms of this Release and bring a lawsuit against any Releasee in violation of the terms hereof, I shall be liable for Releasees’ damages, INCLUDING, BUT NOT LIMITED TO, ATTORNEY FEES.
- I hereby authorize OA Defense to take photographic and videographic images of me and any minor child name below without further compensation. I irrevocably grant the unrestricted right to use and publish photographs and video graphs of me and any minor child identified below, for trade, advertising, and any other purpose or in any manner or medium and to alter and compose the same without restriction and without my inspection or approval. I hereby release to OA Defense and its assigns all claims and liability relating to any such photographs or video graphs. I HAVE READ THIS RELEASE, UNDERSTAND IT, AND SIGN IT VOLUNTARILY AS MY OWN FREE ACT. In witness whereof, I have hereunto set my hand and seal this
