For Military/Veterans – Please upload a scanned copy of your drivers license or state-issued photo ID card. NOTE: Federal law prohibits the scanning or photocopying of an active duty military or federal government ID card. If your drivers license does NOT indicate veteran status we will need ONE of the following additional documents: 1.) Copy of your CAC/LEO Credentials 2.) Copy of a Veteran/Retired ID Card Star ID with “Veteran” indication 3.) A DD-214 (redact sensitive info & must show Honorable discharge) 4.) A memo from your Chain of Command (most current) stating your service and standing 5.) A response to this email from an @mail.mil or .gov account

For Law Enforcement Officers, active-duty or retired – Please upload a scanned copy of your drivers license or state-issued photo ID card, plus a valid law enforcement ID card.

For EMT or Paramedic – Please upload a scanned copy of your drivers license or state-issued photo ID card plus ONE of the following documents: 1.) A valid EMT/Paramedic ID card 2.) A copy of a valid, government-issued certification showing your EMT/Paramedic status

For Firefighters – Please upload a scanned copy of your drivers license or state-issued photo ID card plus ONE of the following: 1.) valid firefighter ID card 2.) copy of a valid and professionally-issued certification illustrating firefighter status
